The residential aged care journey, step 3 of 6

Every resident seen whole, on one clinical record

Care plans, medicines, assessments and incidents share one record, so the nurse on shift sees the whole resident. The AN-ACC class refreshes from Services Australia every morning.

An AI-assisted care handover for an aged care resident in HealthOS: the alerts first, then the policies the draft applied, then the ISBAR sections for a clinician to review and edit.
Who takes part

The people at this step

The registered nurse

Owns the clinical moments: assessments, the care plan, escalation and the handover at the end of the shift.

The care workers

Record the care they give, in the same record the nurse reads.

The GP

Reviews and prescribes against the resident's chart rather than a faxed summary.

What happens

Step by step

  1. A care plan from the assessments

Falls, skin, continence and the other assessments feed the resident's care plan.

  2. Medicines on the chart

Each dose on the medication round is recorded against the chart it was given from.

  3. Changes picked up early

Chronic condition pathways and connected devices pick up deterioration before it becomes an emergency.

  4. Handover

A care handover is drafted from the record for the nurse to review before it is shared, alerts first.

  5. The class, kept current

AN-ACC classes refresh from ACWS daily, with the date each took effect.

What the next step gets

Care minutes and the Quality Indicators read from the care that was recorded, with nothing extracted by hand.
