The Support at Home journey, step 5 of 6

Watching over the client between visits

People living at home are most vulnerable between visits. Auto-syncing connected devices send vital signs to the record, so a change can be picked up before the next scheduled visit.

A home care client's wearable vitals in HealthOS: an active alert for a rising systolic blood pressure above its threshold, with trends for weight, blood pressure and glucose.
Who takes part

The people at this step

The remote monitoring nurse

Sees the alert when a reading crosses its threshold, and follows it up.

The nurse practitioner

Reviews the client by video and changes the plan when the readings call for it.

The client

Uses a scale, a cuff or a glucose sensor with no phone set-up required.

What happens

Step by step

  1. Readings arrive on their own

Connected devices sync to the clinical record without the client doing anything.

  2. A threshold is crossed

Clinical threshold alerting for chronic conditions raises the change for the care team.

  3. A review by video

The client is seen by video from home, on the same record.

  4. Prevention built in

Screening is part of the client's care pathway, at entry and at set intervals.

What the next step gets

The care given between visits is on the same record the month-end claim reads.
